Samaritan Fund Application

The account of the Good Samaritan is found in Luke 10, where Jesus instructed His followers to care for their neighbor.

Park Street Brethren Church actively equips our members and attendees to live out their faith. This involves caring for and leading their families, representing Christ in their workspace, and being representatives of Christ as they lead in their community.

Together as a church, we support one another's love for each other's neighbor and strive to support the Good Work that Jesus is calling each of us to do.

 

Things to know:

  1. Park Street Brethren Church partners with many agencies in Ashland, OH specializing in helping individuals and families in need. Please contact these organizations about your request, as our application will inquire about the support you have received from other entities. Please keep note of individuals who have spoken with if possible.
  2. The most meaningful way to be approved for assistance is to complete this application with a member or attendee of Park Street Brethren Church who is helping you navigate your current time of need. We like to see that others are helping you, caring for you, challenging you, and supporting you in your time of need. 
  3. The Samaritan Fund assists in short-term situations. Monthly stipends to help with living expenses are not available from this Fund.
  4. Most approved requests are to support rent or utility expenses. Information (account number and address) will be required as a part of the application. 
  5. Distributed funds are considered a one-time gift to the assisted individual.
  6. The application involves four (4) sections. The four sections are:
    1. Section 1: Contact and family information 
    2. Section 2: Overview of household income and expenses
    3. Section 3: What caused the problem and how much are you requesting
    4. Section 4: If approved; Provide the account number and address to the individual/business in which you are seeking support

 

Ohio - The state prevents disconnection under certain conditions from October to March. Other rules state that disconnection is prohibited for Percentage of Income Plan (PIP) customers as long as they remain current with their PIPP payment. Another option, the winter reconnect order allows residential customers who are disconnected or being threatened with disconnection the opportunity to pay no more than $175 to maintain their utility or heating service. If the customer’s service has already been disconnected by this time, the customer must pay a reconnect fee of no more than $36 to restore their service. Customers who utilize the Winter Reconnect Order must enter into an extended payment plan on their remaining balance. Customers requesting new natural gas or electric services, who have no previous balance with their utility, may establish service under the winter reconnect order by paying $175, rather than paying the required security deposit. Another regulation provides for a 30 day disconnect delay if dangerous to health or if medical or life support equipment is necessary — as certified by a doctor or medical professional. If the customer has been shutoff, and the form is submitted within twenty one days of the shutoff, service is restored for the customer with the condition.
